Covid death: Application for appeal and certificate from tomorrow; How to Apply?

Those who do not know how to apply online can apply online through PHCs or Akshaya Centers by providing the required documents. Application through Covid-19 Death Info Portal
Application for death diagnosis and certificate can be made through e-HealthCovid19 Death Info Portal (https://covid19.kerala.gov.in/deathinfo). How to apply?
